What is Chronic Care Management (CCM)

Chronic Care Management (CCM) is a Medicare-supported service designed for patients living with two or more chronic conditions expected to last at least 12 months or longer, such as diabetes, hypertension, COPD, or heart disease. Elevate Healthcare's goal is to help improve overall health outcomes, reduce hospitalizations, and ensure patients receive consistent, coordinated care tailored to their individual needs. Participation is voluntary and services are provided in accordance with CMS guidelines.
